Abstract:

Kaiser Permanente (KP) Southern California is an integrated health system serving approximately 4.5 million members. KP has a rich history of collecting and analyzing patient health data to support preventative care and improve quality. A primary goal of KP’s Health Innovation department is to improve both health outcomes and health system efficiency by enabling personalized, proactive care through the application of advances in data science and machine learning to EHR data. I will present two case studies that illustrate our methodology including use case selection, data acquisition, model development, implementation, and evaluation. One initiative involves pairing a predictive model that identifies likely high-utilizers years in advance with interventions that address these members’ clinical and psychosocial needs.  The second use case is a broader program to develop predictive tools and recommendation engines to enhance proven chronic disease management programs.
